If you would like to use this form, you may print it and then fill it out or you can fill it out on your computer and then print it. After printing it, send order with cash, check, or money order in U.S. funds via U. S. mail. Sorry, no C.O.D.s or credit cards. We do accept PayPal payments payable to rsimpson@141.com.

Name:		For each country you want to have custom printed, specify the country name, the time period the pages are to cover, and the paper on which you want the pages printed. If you do not specify a paper size, 8½" x 11" paper will be used. Telephone number and/or email address will be used if we have questions about your order.

Media Mail shipments can take 1 to 10 weeks. Priority Mail shipping takes 3 to 5 days and is available for $11.50 for up to 300 pages. Otherwise, contact me for a quote. All shipping is at the buyer's risk. For insurance, add $1.50, which includes USPS Delivery Confirmation.
