If you would like to use this form, you may print it and then fill it out or you can fill it out on your computer and then print it. After printing it, send order with cash, check, or money order in U. S. funds via U. S. mail. Sorry, no C.O.D.s

Media Mail shipments can take 1 to 10 weeks. Priority Mail shipping takes 3 to 5 days and is available for $9.80 for up to 300 pages specialized or international, 500 pages 8½" x 11". Otherwise, Contact me for a quote. All shipping is at the buyer's risk. For insurance, add $1.50, which includes USPS Delivery Confirmation.
